If you contact us before discharge, we aim to complete the assessment and have care ready on the first day home. Tell us the expected discharge date and we'll plan around it.
Can we keep the same caregiver?
Yes, consistency is part of the plan. We match a caregiver to your loved one's condition and track caregiver consistency in the monthly scorecard.
What happens in an emergency?
Every care plan has a clear escalation process. The caregiver follows it, your Care Manager is alerted, and the family is informed at once. In a medical emergency, we call 995 first.
How much does it cost?
Packages are priced monthly and depend on the hours and services needed. We'll explain the costs clearly at your free consultation, with no obligation.
